management or monitoring precaution: Renal- Adjust starting dose and do not exceed a maximum of 2 mg/day for CrCL<60 mL/min. Hepatic-Monitor hepatic transaminases. Musculoskeletal-Elderly patients greater than age 65 may be at increased risk for myopathy and rhabdomyolysis.
