Does vertigo frequently interfere with your normal activities?

Do you frequently have vertigo that has not been checked by your doctor?

Have you had vertigo for longer than 5 days?

Do you have lightheadedness that is getting worse, but you do not have signs of shock?

Do you have moderate to severe vertigo following a head injury?

Do you have persistent mild vertigo following a head injury?

Have you had mild to moderate vertigo for 24 hours or longer after an ear injury?

Have you had lightheadedness that comes and goes for 2 to 3 days after an injury?

Have you had mild persistent vertigo for 2 to 3 days after an injury?

Have you fainted more than 1 time when trying to stand up?

Have you had more than 4 episodes of moderate to severe lightheadedness in the past 24 hours?
