If you are PREGNANT

If you are an adult over 60
- management or monitoring precaution: Renal-Elderly have been shown to have increased serum drug concentrations. This may lead to increased risk for hemorrhage. Both thrombotic and bleeding event rates were higher in older patients, but the risk-benefit profile remained favorable. Assess renal function before initiating therapy. Reduce dose in patients with CrCl=30-50 mL/min. Avoid use if CrCL< 30 mL/min for VTE treatment or prophylaxis; avoid if CrCL < 15 mL/min for nonvalvular atrial fibrillation.
