Any care or treatment a health plan pays toward or pays for in full is a covered service.
Before you choose a health plan, look at its summary of benefits so you know what is and isn't covered. Also, check for the amount you have to pay for a covered service.
If a health service is not covered by your plan, you may be responsible for all the costs associated with it. Even if a treatment is covered, you may have to see a network provider in order for the health plan to pay for it, get a referral from your primary care provider, or get pre-approval from your health plan.
