  • Precaution: CONTRAINDICATED IN G6PD DEFICIENT INFANTS AND <1 MO D/T HEMOLYTIC ANEMIA RISK

    • If you are an adult over 60

  • management or monitoring precaution: Renal-Beers Criteria contraindicates use in CrCL<30mL/min. due to increased risk for neuropathy, hepatic/pulmonary fibrosis. Neuro/Psych-Peripheral neuropathy risk particularly with long term UTI suppression and renal dysfunction. Pulmonary-Interstitial pneumonitis/fibrosis risk. Hepatic-Case reports of severe hepatotoxicity in the elderly on long term UTI suppressive therapy.

    • Pediatric use

  • Severe Precaution: May cause hemolytic anemia in neonates under 1 month of age.
