Limitations of Use of the COPD-PS:
By using the COPD-PS tool, the user (“User”) understands and agrees that: (1) User may make paper copies of and use the paper version of the COPD-PS tool with patients in User's practice at no cost to User; (2) User may electronically store a PDF version of the COPD-PS tool and print out such version for use with patients as part of User’s practice at no cost to User; (3) when completed by patients as part of the User's practice, User may scan and store copies of completed COPD-PS tools in the User's paper or electronic medical record at no cost to User. To ensure that the validation of the screener is maintained, User shall not and shall not cause any third party to modify or amend the COPD-PS tool in any manner without the prior written consent of OptumInsight Life Sciences, Inc. (f/k/a “QualityMetric Incorporated”) (“QM”). User understands and agrees that any such modification or amendment may affect the validation of the COPD-PS and QM shall not be responsible for the results of the COPD-PS or any validation thereof pursuant to a modified or amended COPD-PS.
Inclusion in Electronic Health Record Systems:
Physicians, group practices and other authorized Users, as applicable, may incorporate the COPD-PS tool into their own electronic health record systems for use solely with patients in their own practice at no cost to such User as permitted in item (2) of the previous paragraph or otherwise incorporate the COPD-PS tool into User’s electronic health records system so long as it is for use solely with patients within User’s own practice. To determine whether any other electronic use is permitted, and to ensure that validation of the screener is maintained, User is encouraged to contact QM before incorporating the COPD-PS tool into the User’s electronic health record system other than as permitted in item (2) above or by scanning completed copies of the tool. User shall not and shall not cause others to include the COPD-PS tool into an electronic health record system for commercial benefit (i.e., shall not and shall not authorize or permit any third party to commercialize, lease or sell any electronic version of, or any electronic medical record system containing, the COPD-PS tool) without written consent from QM.
